Who we work with

The Arbor team work with adults and older children in a community setting who have an acquired brain injury. This includes a wide range of presentations, including traumatic brain injuries, strokes, other complex neurological conditions and degenerative neurological diagnoses, including dementia.

Referral pathway

A typical process when you work with Arbor

Initial referral received

A referral is submitted outlining the client’s rehabilitation needs and background information.

Clinical contact

An Advanced Clinical Practitioner or Therapy Lead contacts the referrer by phone or email to clarify the client’s presentation, goals, and any immediate priorities.

Cost proposal and associate profiles

A detailed cost proposal is provided within 48 hours, including relevant associate CVs and an outline of the proposed intervention plan.

Funding agreement

Once funding is confirmed, therapy input is scheduled in collaboration with the referrer and client team.

Initial assessment

A comprehensive assessment is completed, followed by a summary report detailing clinical formulation, recommendations, and projected costs for the initial three-month intervention period.

Case formulation and MDT goal-setting

The multi-disciplinary team (MDT) collaborates to establish client-centred, measurable goals to guide the intervention phase.
